Not a copy of it. Not a portal onto ours. The record itself.
Your intake, your consents, your Good Faith Exam, and the note and photographs from every visit are filed into a vault on MeCentral that belongs to you. Not to Angelica Puente Aesthetics. This practice holds the schedule, the money and the stock cupboard; your medical record sits on your side of that line.
If somebody breaks into this practice's computers, they get appointment times and billing. They do not get charts, photographs or medical history, because those are not here. That is a genuinely smaller thing to lose, and it is worth saying plainly rather than burying in a privacy policy.
When you complete your intake you ask this practice to be the custodian of what it writes about you: to keep it current, correct it, and send it on your behalf when you ask. That is a permission you give, and you can take it back at any time from your records page, without giving a reason.
Taking it back deletes nothing and moves nothing. Your records were already yours; what stops is the practice holding and sending on your behalf.
A supervising physician reviews some charts. When that happens they get a link to one chart — that one, nothing else — which expires on its own and stops working the moment they sign off. They do not get an account here and they cannot browse. Their sign-off is filed into your record as a document of its own, with their name on it.
Your records page lists every one of those links while it is live, and you can see when it expires.
Before-and-after photographs are taken at every visit as part of your medical record, whether or not you ever agree to anything being published. Using them in marketing is a completely separate form, it is specific, and you can limit it or decline it. Declining changes nothing about your care, your scheduling or your pricing.
Nothing happens to your records. They are already yours. You can hand them to your next injector without asking us, and you do not need this practice to still exist for that to work.
Your records are not stored by the practice. They live in a vault that belongs to you, on MeCentral, and they go where you go — to a new injector, to another city, or nowhere at all. If this practice closed tomorrow you would still have them.
